INITIAL VISIT- All new patients are required to schedule their 1st visit as an INITIAL. Please print out the forms to the right for either a regular patient or Motor Vehicle/Personal Injury/Workers Compensation.
Extended & Regular Visits- Once an Initial Visit has been completed, patients can chose from an Extended Visit (50 min) or a Regular Visit (20 min). Special rates given for 18U & 65+
Accepted Payments: Cash, Checks,
Credit Cards, HSA, Venmo, Zelle, & Paypal (all credit usage will include a 4% service charge)

Mon 1-7, Tues 9-12, 3-6 Wed 1-7 Thurs 3-7 (alternating)
Home Office New Patients Paperwork- please fill out the attached initial paperwork by downloading the forms & returning to Doc MC during your Initial visit.
Motor Vehicle Accident/Workers Compensation Patients- please download, complete & return your initial paperwork to Doc MC during your Initial visit.
